What does chronic care management do?

Chronic care management (CCM) is a specific service that provides coverage for patients with two or more chronic conditions for a continuous relationship with their Douglas Young, M.D., care team. Under CCM, the patient's care team can bill for time spent managing the patient's needs and health conditions.

Chronic care management coordination is done outside of regular office visits for patients with two or more chronic conditions predicted to last at least 12 months or until the patient's death. These conditions or diseases place the patient at significant risk of death, functional decline, decompensation, or acute exacerbation.

(CCM) provides ongoing support and is an excellent option for patients who need regular help and guidance to manage chronic health conditions.

What is remote chronic care management?

Remote chronic care management (remote CCM) remotely manages patients' chronic health conditions and diseases. It’s often referred to as remote patient monitoring (RPM), which monitors patients’ health remotely.

RPM and CCM use technologies to build the bridge between doctor visits. By using remote patient monitoring to manage chronic conditions, doctors and patients can reduce the number of in-person visits. This makes care more accessible as the patient doesn’t need to worry about driving or coordinating transportation to receive care.

What are some examples of chronic conditions?

A chronic condition is a disease that develops with time or a health condition that is persistent and long-lasting in its effects. The term chronic is often applied when the health condition lasts three months or more.
